Community care

Our Community Palliative Care Team consists of specialist nurses who work with your GP, District Nurses and other colleagues to support and advise you whilst you are at home. We will also visit if you are in a nursing home, residential home or local community hospital.

We have specialist knowledge and experience in palliative care and offer advice about managing symptoms that may arise, as well as being able to give emotional support to you and your family.

You will be allocated a Community Nurse Specialist on referral, and she will be your named contact with the hospice.

The amount of contact with your Nurse Specialist will vary according to your need, and will be discussed with you and your family. You can choose to be visited at home or be seen at the hospice Outpatient Clinic.
