The Hospice at Home Team aims to offer general hands on nursing care for patients requiring end of life care in their own homes. Working closely with Community Nurses and GPs, the Team of Registered Nurses and Healthcare Assistants can help by providing support with personal care, mouth care, wound care and basic symptom control.
The Team is based at St Barnabas House and works from 8.30am to 6pm Monday to Friday, and 8.30am to 4.30pm Saturday and Sunday. This service aims to offer personal, high quality care for patients in their own homes and has been set up to provide a stronger choice for patients requiring end of life care, and who are registered with a GP across Arun, Adur and Worthing.
Referrals are accepted direct from your Primary Care Health Team, for example, your GP or district nurse. Referrals to the service can be made between 8.30am and 4.30pm Monday to Friday. The service is not an emergency response service and the Team will endeavour to respond to urgent requests as soon as is possible.
In addition to nursing care, the Team can also provide respite care for up to 3 hours during weekday afternoons. This can be booked in advance through the Hospice at Home office. The aim is to give carers a short break in looking after their loved ones.
