Who to refer

Referrals are accepted for patients with active, progressive and advanced disease (malignant and non-malignant) with a limited prognosis. For non-malignant disease, the estimated prognosis should be less than one year. The Gold Standards Framework Prognostic Indicators can assist with identification of such patients.

For specialist medical and nursing advice and intervention, patients should have a level of need that requires additional support to pre-existing primary or secondary care.

For Hospice at Home care and support, patients should have a level of need that requires physical nursing care whilst approaching the last few days/weeks of life.

How to refer

Any healthcare professional may refer but the referrer must inform the patient’s GP. The patient or their representative must be aware a referral has been made and must give consent. If a patient is not aware, there may be delays in processing the referral.

Please click on the link below to download our referral form. These can be faxed to the hospice on 01903 706396.

Responding to a referral

All patients referred will be contacted within two days unless otherwise stated on the referral form.

Please note that referrals for the Hospice at Home service only will be contacted on the same day, referrals for urgent specialist advice and support will be seen within two days and referrals for non urgent specialist advice and support will be seen within 10 working days or an alternative time scale, according to patient’s wishes.

If you need urgent specialist advice, please telephone St Barnabas House Community Team on 01903 706349 or 01903 706350.
