Advance Care Planning (ACP) has the potential to holistically benefit a service user and their loved ones to the point, and beyond, their death.

Learning outcomes

• What is ACP and why is it important
• To identify potential barriers to engaging in ACP
• To consider legal aspects of ACP
• To explore how to initiate and support conversations relating to ACP
• To reflect upon our own practice in relation to ACP and difficult conversations
