Patients may be referred to St Barnabas House for management of symptoms, emotional support, counselling, Day Hospice support, out-patient support and palliative or end of life care.
Referrals are accepted for patients with active, progressive and advanced disease (malignant and non-malignant) with a limited prognosis. For non-malignant disease, the estimated prognosis should be less than one year. The Gold Standards Framework Prognostic Indicators can assist with identification of such patients.
For specialist medical and nursing advice and intervention, patients should have a level of need that requires additional support to pre-existing primary or secondary care.
For Hospice at Home care and support, patients should have a level of need that requires physical nursing care whilst approaching the last few days/weeks of life.
Any healthcare professional may refer but the referrer must inform the patient’s GP. The patient or their representative must be aware a referral has been made and must give consent. If a patient is not aware, there will be delays in processing the referral.
