Living Well Coordinator

Hours: 22.5 hours pw (job share)
Salary: £13,728 - £14,772 p.a. depending on depth and variety of experience in a similar role (F.T.E. £22,880-£24,620)
Closing date: Sunday 31 October

St Barnabas House provides specialist palliative care to adults in the Worthing, Adur, Arun and Henfield areas through a range of services, including in-patient care and community living well services.

Are you someone who is passionate about helping patients to be proactive in Living Well? We are looking for an enthusiastic person to provide excellent support to our new Living Well service and well-established befriending service, Community Companions. You will have excellent organisation skills, have a calm and measured approach to problems and be a good team player.

This role is a job share and requires the applicant to be able to work Wednesday – Friday

 

This role involves:

  • Supporting Community Companions in their voluntary role of visiting patients in the community.
  • Assisting the organisation and delivery of Community Companions training to equip volunteers to be confident in their role and co-facilitating the running of group supervision meetings for volunteers (online and face to face)
  • Managing the Community Companions referral process, liaising with referrers, volunteers and patients.
  • Providing practical support to ensure the smooth running of group sessions (online and face to face)
  • Maintaining the profile of the Living Well and Community Companions service internally and externally by working with the marketing department
  • Liaising with Voluntary Services colleagues regarding the recruitment of new volunteers to support the Living Well Service
  • Assisting the Living Well Manager to develop the new service including linking with community groups across the St Barnabas catchment area
  • Input, collate and produce statistical data and analytical information and report effectively, in particular relating to the effective evaluation of the service.
