30 hours per week

Salary

£24,880- £28,752 p.a. depending on depth and variety of  experience in a similar role (F.T.E. £31,100 – £35,940)

About the role

We are looking for an experienced social worker who will provide a social work service to patients and their families, particularly in the community and mostly with adults, working closely with other members of the Family Services Team and the hospice’s multi-professional team.

You will need to be a confident and competent social worker with excellent verbal communication skills and the ability to work with adults experiencing change, loss and bereavement:

  • Undertaking psychosocial assessments of the needs and resources of patients and their family members
  • Providing ongoing, advice and information to patients and their families
  • Work with individuals and other family groupings

Liaising with other care providers and organisations for the benefit of your clients
