What is Living Well?

We understand how difficult, overwhelming and lonely being diagnosed can sometimes be and want to support you to be living well every day, while recognising and managing the challenges you may face. Living Well has been designed to help you to get moving, enjoy new activities and socialise, all in a supportive safe environment. We also recognise that being a carer of someone with a life-limiting illness also has its own unique set of challenges, and we’ve expanded our services to help support carers too.

Who can access the service?

Our Living Well services are open to anyone over the age of 18 in our catchment area and who is also one of the following:

  • someone with a life-limiting or terminal illness (please see the FAQs for more information and eligibility)
  • someone living with Frailty
  • someone who is a carer of someone with Frailty or a life-limiting or terminal illness

A life-limiting illness is any illness or condition which:

  • cannot be cured and
  • is likely to lead to someone’s death.

You might hear other terms like ‘terminal’ illness or diagnosis, which means the same thing.

Anyone can be diagnosed with a life-limiting illness, and we accept any aged person over 18 to come to Living Well.

There’s lots of examples of life-limiting illnesses, including:

  • frailty
  • advanced cancer
  • dementia (including Alzheimer’s)
  • kidney failure
  • lung disease
  • neurological diseases, like Parkinson’s
  • advanced heart disease.

This is not an exhaustive list and you might have another life-limiting illness not listed. If you’re not sure if you have a life-limiting illness, please fill in the form and we’ll get back to you to discuss.

Frailty is a distinctive health condition related to ageing, describing a general decline in function and reserve across multiple body systems. People with frailty may struggle with independent living, find everyday tasks more difficult, and experience a gradual reduction in social interactions.

We have received special funding to trial a new programme for anyone in the catchment area with frailty. Each session of the six-week course will focus on an aspect of frailty: fatigue; exercise; nutrition; emotional health; planning ahead and financial support. You’ll be provided with top tips and ideas on living with frailty and after the six-week programme, there’s the option to dip in and out of the Living Well service.

Carers have told us how beneficial they have found attending the Wellbeing Programme. By getting involved you’ll be gaining knowledge on issues that affect your loved one with a life limiting illness and you’ll have the opportunity to share with others in a similar situation.

The Wellbeing Programme lasts six weeks and we meet for 1.5 hours every week.

You can sign up to any Living Well session if you are:

  • Over the age of 18
  • Live in our catchment area (Worthing, Adur, Arun and Henfield)
  • Have a life-limiting illness (see the definition above)
  • Are a carer for someone who fits the above criteria

You don’t need a GP to refer you to our Living Well service – just fill in the form below and we’ll discuss what sessions might be best for you.

All sessions are held at St Barnabas House in Worthing. Please be aware that all sessions take place at St Barnabas House and in order to take part in groups and appointments you must be able to travel to and from the hospice. You can find out more about visiting the hospice here.
