Disease specific care services

We have created a new specialist nursing service with four dedicated Nurse Specialists working in key areas of care to support patients with End Stage Renal Failure, End Stage Liver Disease, Advanced Heart Failure and Dementia.

As part of our Hospice Care Pioneers developments we have introduced a team of specialist nurses who will support patients with other end-stage conditions, as well as cancer.

Renal Supportive Care Service

The Renal Supportive Care Nurse aims to provide a high quality service to manage the symptoms and provide support for psychological and emotional needs that may occur as a result of Advanced Kidney Disease (End Stage Renal Failure).

The Liver Supportive Care Nurse aims to provide a high quality service to manage the symptoms and provide support for psychological and emotional needs that may occur as a result of End Stage Liver Disease.

Dementia Supportive Care

The role of the Dementia Lead Nurse is to adapt our existing services in order to support patients with a diagnosis of Dementia, and also educating staff and other healthcare professionals to be able to provide care specific to the needs of these patients.
